Certification Levels


QOPI® Certification- Pending

  1. Applied for QCP and submitted initial documentation.
  2. Until QOPI Certification is awarded or expires; within 12-14 months, if QOPI Certification is not achieved.

QOPI® Certification

1. Met or exceeded QOPI® Certification Measure scoring requirements in one or more QOPI rounds

2. Met QOPI Site Assessment safety standards.

3. Participated in QOPI abstraction at least once per year during three year Certification term.

For three years or until renewal, as long as Certification maintenance requirements are met.

Onsite Survey

Certification Survey

All applicant practices are subject to an on-site survey. On-site surveys will be conducted by an experienced oncology certified nurse. The nurse will select and follow patients through chemotherapy administration, examining records, policies, and tools to evaluate all 20 Certification standards.

Before the Visit

Your on-site survey will be scheduled following your submission of chart and standard documentation. You will receive a notice from QOPI Certification staff which will provide you information on the on-site surveyor selected for your practice. Surveyors are chosen in compliance with ASCO and QCP's Conflict of Interest Policy; we ask that your practice confirms that there is no conflict of interest with the selected surveyor. Once this process is complete, your surveyor will contact you directly to find a mutually agreeable date for your survey.

After the Visit

After your on-site survey, your surveyor will write a report which will detail your practice’s concordance with the 20 Certification safety standards, including any requirements for additional documentation. This report is reviewed by QCP staff and then sent to a member of the QOPI Steering Group or QOPI Certification Program Oversight Council for final review and approval.

Your practice should receive its report 4-6 weeks following your on-site visit. Once you receive the report, you will have 10 business days to submit a plan to address any standards deficiencies found during your survey.

Once your plan has been approved, your timeline to submit any additional documentation will depend on your practice’s Certification status. For recertifying practices, your responses will be due 4 weeks prior to the end of the 90 day grace period following your Certification term end date. For new practices, your responses will be due 4 weeks prior to the one-year anniversary of your Certification Pending date.

If a repeated site visit is required to demonstrate practice improvement, the practice will bear the cost of the review including travel and stipends.

QCP On-Site Review – A Day in the Life

Introductions/brief tour of facility/overview of on-site review activities for the day

1-2 hours [depending on the flow of the chemotherapy infusion room and the timing of chemotherapy preparation]

Review of chemotherapy orders and observation of chemotherapy order checking.

Observation of chemotherapy preparation and labeling.

Observation of double check of prepared chemotherapy.

2-4 hours

Review of chart documentation for two patients receiving IV chemotherapy and two patients receiving oral chemotherapy (on-site reviewer will select records to review). If possible, this review should occur in an area away from the pharmacy or patient care areas, and a staff member familiar with the EMR/paper chart is needed to assist in locating needed items.

Review and/or discussion of policies and procedures for the 20 QCP standards.

Review of pharmacy and nursing personnel records (licensure, orientation/training, etc.).

1 hour [10-15 minutes each]

Interview of 2-3 nurses who administer chemotherapy. Additional interviews of the nurse educator and administrative staff may be requested.  Interviews will occur at a mutually convenient time in a private area away from the patient care area. One nurse is interviewed at a time.

Interview of personnel who prepare chemotherapy and/or dispense oral chemotherapy (when applicable).

Reviewer in quiet space alone to review notes.

Certification Maintenance

To maintain QOPI Certification status, all QOPI Certified Practices must participate in QOPI chart abstraction by submitting data for at least all core measures plus one module of your choice once per year, based on your Certification date.

For example, if your practice is Certified in April 2015, you would need to participate in QOPI to maintain your Certified status in either Fall 2015 or Spring 2016. You’d then have to participate once more in either Fall 2016 or Spring 2017. Then, you would abstract the full five Certification modules in Fall 2017 to begin the recertification process. 

There is no need to register or apply for maintenance; the QOPI Certification team tracks your participation. Additionally, there is no scoring requirement for your maintenance rounds, however your practice will receive an interim and final report. While the requirement is only one module, we encourage you to abstract as much as you’d like for your own quality measurement purposes.

If the practice does not participate in QOPI chart abstraction once per year they will receive a warning notice. After one warning notice has been sent, the practice must participate in the next QOPI chart abstraction process or the practice will lose their Certification status.

Become Re-Certified

Continue your commitment to Excellence through Re-Certification

The QOPI Certification term is three years from the date your practice earned your  QOPI Certification award. Your Practice must apply in advance of your term ending date in order to maintain a continuous Certification designation.

 Participate in a QOPI Chart Abstraction round about 9-12 months prior to your term expiration. This will allow your practice to apply for Certification in the QCP application period opening just after the abstraction period closes  (spring or fall) and will ensure your practice achieves re-certification prior to the end of your current term. As long as you complete the renewal process prior to your term end date, you will always retain your original Certification date with the term extended another 3 years.

Complete Spring QOPI Abstraction by May 19th

Practice notified with instructions on submitting  randomly selected charts and standards (You will have 4 weeks to Submit).

After Charts and Standards are confirmed to be submitted you will achieve  Recertification pending status; You will receive a conflict of interest questionnaire regarding the assigned surveyor.

We will then assign a surveyor and send the conflict of interest questionnaire.

Late July-Mid August  2015

Practice is notified of surveyor and physician reviewer  within 2 weeks of pending status and surveyor given permission to contact practice

Late July –Early September 2015

Surveyor will work with your practice to schedule survey, confirm date  (within 4-6 weeks) and complete the On Site Survey.

4 weeks post survey

On site review report will be sent to practice and list any requirements

Mid February  2016

Practice requirements are due to be reviewed and approved. Your practice can submit responses any time after the review but the last date they can be submitted will be February to allow us time to review.

March-Early April  2016

Re-Certification is Awarded for current term

(Recertifying practices are encouraged to apply 9-12 months prior to their current term ending)
